    Huge Thank You

    Surv catered a very special celebration for our family, and they exceeded every expectation! From the moment I booked with Janice, the entire process was seamless. She was incredibly helpful, organized, and made sure every detail was taken care of. The food was absolutely amazing—fresh, flavorful, and beautifully presented. Our guests couldn’t stop talking about how delicious everything was, and there was something for everyone to enjoy. The quality of the food and the professionalism of the team truly made our special day even more memorable. A huge thank you to Janice and everyone at Surv for helping make our celebration such a success. I highly recommend Surv for any catered event and will definitely be using them again!

    Wonderful Event

    This was a wonderful place to host our celebration dinner, they where willing to work with our dietary needs of a few of our guests. The cocktails where delicious. It is a nice option in Forks township

• A valid credit card is required to confirm your reservation. No deposit is required. • Cancellations made within 30 days of the event will incur a $500 cancellation fee. • If your event is rescheduled within 30 days of the original event date, the cancellation fee will be applied as a credit toward your final bill.
SURV welcomes celebrations year-round and does not have a designated peak wedding season. Rather than seasonal pricing, our food and beverage minimums are based on the day of the week. For full restaurant buyouts, Friday and Saturday events require a higher food and beverage minimum due to demand. Holiday events may also be subject to increased minimums. Our private dining room, however, does not have seasonal or weekend minimum increases. We believe in providing a fair and flexible experience for our couples, making it easier to celebrate with family and friends without unnecessary pricing adjustments.
Yes, all our facilities are wheelchair accessible. We also have ramps at the main and side entrances, hearing-impaired fire alarm lights, and trained staff.
Complimentary parking is available behind the shopping center. We recommend using the main entrance located on the side of the building, which is fully handicap accessible.
Guests are able to bring their own wine, with prior acknowledgement of our Event coordinator we do charge a $25.00 per bottle cork fee.
